Job summary

Endurance Home Care is hiring per diem RNs in New Jersey to perform in-home assessments and develop care plans for clients. You will work under the Nurse Supervisor and visit clients in Union, Essex, Bergen, Passaic, Morris and other counties.

Responsibilities include initial assessments, 60-day supervisory visits, and coordinating with CHHAs. A NJ RN license and at least 1 year of clinical experience are required, with mileage reimbursement and flexible scheduling offered.

Qualifications

• Active New Jersey RN license.
• At least 1 year of clinical experience; home care, elder care or community health preferred.
• Reliable transportation and a valid driver’s license.
• Comfortable doing independent in-home visits and documenting electronically.

Responsibilities

• Perform in-home nursing assessments for new clients.
• Create individualized care plans based on client needs and home environment.
• Meet with clients and families to explain the plan of care and answer questions.
• Assist in matching clients with appropriate caregiver.
• Conduct 60-day supervisory visits to review client conditions and update care plans and documentation.
