Job summary

Signify Health, part of CVS Health, seeks a Per Diem Physician to perform in-home health evaluations, connecting members with the right care and bridging gaps in care. Visits are at members’ homes, often ~45 minutes, with travel to licensed states and virtual options as needed.

You will use an iPad and clinical workflow, conduct health reviews, and provide diagnostic services as directed. This is a part-time, per diem role with flexible scheduling and a base hourly rate.

Qualifications

• Active, unrestricted medical license in coverage area.
• Multi-state licenses preferred.

Responsibilities

• Visit patients in their homes for health evaluations and questions.
• Connect patients with appropriate care within health plan networks.
• Work with Signify Health clinicians to deliver in-home assessments.
• Travel locally and within licensed states as needed.
• Conduct virtual visits when required.
• Provide Diagnostic & Preventive Services as directed (e.g., spirometry, DEE, bone density).
