Job summary

Connect Pediatrics in Frisco, TX is seeking a Pediatric Speech-Language Pathologist (SLP) or SLP Assistant for in-home therapy across Frisco, Little Elm, Aubrey and surrounding areas. We build strong relationships with families to deliver coordinated care at home.

You will perform evaluations, develop and implement treatment plans, provide in-home therapy for pediatric patients, and communicate progress with families while collaborating with a multidisciplinary team.

Responsibilities

• Perform comprehensive evaluations, using standardized tests to determine pediatric patients' needs for therapy. (SLP only)
• Use your clinical judgment to develop and implement relevant treatment plans aligned with physician's orders.(SLP only)
• Provide in-home therapy services for pediatric patients with a wide range of diagnoses.
• Engage with patients' families, providing them with regular communication and updates on goals for their child.
• Collaborate with a multidisciplinary team, including other therapy disciplines, physicians, and nurses, to develop holistic treatment plans that address the diverse needs of each patient
• Maintain comprehensive documentation that reflects the care provided and communicates changes in treatment plans to the entire healthcare team.
• Actively participate in one-on-one meetings with team leads to discuss patient progress, share insights, and refine treatment strategies collaboratively.
