Qualifications

• Registered Nurse (RN) license in California.
• Experience in palliative, hospice, or chronic care management.
• Strong communication, empathy, and active listening skills.

Responsibilities

• Assess patient symptoms, pain levels, and overall comfort.
• Develop and adjust care plans to address physical, emotional, and spiritual needs.
• Educate families and patients about palliative care options and coping strategies.
• Coordinate with physicians, social workers, and other providers for comprehensive support.
• Maintain clear documentation and follow regulatory guidelines.
