Description

We're looking for an In-Home Outreach Counselor – Bilingual, English and Spanish to work in the Healthcare Systems and Services industry in Los Angeles, California, United States.

1. Conduct thorough psycho-social assessments using standardized in-home tools and assess the well-being and safety of children within the home environment.
2. Provide in-home counseling, parent education, teaching and demonstration, case management, crisis intervention, child-focused activities, substitute adult role modeling, and supportive services. Equip families with tools to improve overall family functioning.
3. Meet with families regularly to continually assess the family and home for child safety and risk issues as outlined in the MCPC case plan.
4. Complete administrative tasks to track progress and report activities, including timely and accurate documentation such as progress notes, monthly reports, quarterly MCPCs, maintaining client case files, and submitting weekly case notes, monthly reports, and mileage logs to the Program Coordinator by due dates.
