Job summary

Aveanna Healthcare in Nashville, TN seeks an Occupational Therapy Assistant (OTA) for a part-time PRN role to support our home health patients. You will work under an OT, delivering direct therapy in Davidson and Sumner Counties and following the plan of care.

The qualified candidate is a graduate of an OTA program with a current state license, plus strong organization and communication skills, committed to high-quality home health care and patient education to support recovery and independence.

Qualifications

• Graduation from an Occupational Therapist Assistant program from an accredited university/program.
• Current, unrestricted state license as an Occupational Therapy Assistant in the state of practice.
• Demonstrated proficiency in clinical assessments, documentation and compliance with policies and procedures

Responsibilities

• Provides direct occupational therapy under supervision and per plan of care.
• Communicates plan of care updates to patient/caregiver.
• Provides skilled therapy services and treatments as appropriate for patient care and safety.
• Reports observations to supervisors/physician as appropriate.
• Records clinical notes and progress timely to ensure continuity of care.
• Participates in discharge planning and communicates discharge plans to patients and caregivers.
