Job summary

FOX Rehabilitation is seeking a licensed Occupational Therapist to join our Grafton, WI team, delivering one-on-one geriatric care in private homes or senior living communities. You'll evaluate, design plans, and deliver evidence-based therapy, using our Geriatric House Calls model, with flexible scheduling and telehealth options to extend care and support for older adults.

Qualifications

• Valid Occupational Therapy license in the state of practice.
• Ability to work autonomously delivering one-on-one geriatric care.
• Experience with in-home or senior living settings is a plus.

Responsibilities

• Provide one-on-one Occupational therapy to older adults wherever they live.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility, and independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate.
