Lutheran Senior Services is seeking a PRN home health occupational therapist to provide direct OT in patients' homes, receive physician orders, and develop and implement plans of care. The OT will supervise qualified assistants and evaluate service effectiveness, upholding the organization’s Christian mission and core values.

The role involves coordinating with an interdisciplinary team, documenting visits, and ensuring adherence to orders, with on-call requirements for weekends and holidays.

Qualifications

• Valid state licensure to work as an Occupational Therapist.
• Ability to work independently.
• Knowledge of Microsoft Office or equivalent programs.
• Excellent customer service skills.
• Excellent problem resolution skills.
• Detail oriented and good organizational skills.
• Current CPR certification.

Responsibilities

• Assesses patient functional ability and potential; may perform initial home evaluation when doctor orders OT services.
• Completes OT evaluation to determine potential benefit and establish plan of treatment.
• Develops and implements medically prescribed OT plan of treatment.
• Prepares written plan of treatment for physician’s signature, including findings, goals, and rehab potential with specifics on modality use, amount, frequency and duration.
• Interprets medical information from patient records and interdisciplinary team.
• Establishes goals and discharge plans.
• Adapts physician orders for home setting.
• Coordinates treatment plan with interdisciplinary team.
• Instructs and supervises Home Health staff (HHA) who may carry out portions of OT at home.
• Instructs and supervises COTA and evaluates progress at least every 30 days.
• Instructs patient/caregiver/family in OT home care.
• Documents each visit timely and accurately.
• Communicates data and changes in patient condition to physician and team.
• Orders or supervises ordering of specialized durable medical equipment.
• Attends case conferences and agency in-service programs.
• OT may be on call for weekends/holidays on a rotational basis.
