Job summary

Nightingale Visiting Nurses in Stamford/Norwalk seeks a licensed Occupational Therapist to provide in-home evaluations and therapies, helping patients regain independence in daily living. Qualifications

• Licensed Occupational Therapist (OT) in CT required.
• Minimum 1 year OT experience preferred.
• Current TB test and CPR certification; physical within last 12 months.

Responsibilities

• Evaluate and treat patients to improve daily living and work activities.
• Design and implement individualized therapy plans.
• Supervise COTAs and HHAs per care plan.
• Document treatment progress daily and ensure HIPAA compliance.
• Coordinate total patient care with the healthcare team; prepare discharge summaries.
