Job summary

Bronson at Home is seeking a compassionate Occupational Therapist to join our Home Care team on a PRN basis. You will help patients regain independence and improve daily function in the comfort of their homes.

Responsibilities include evaluating patients, creating personalized treatment plans, educating families, and coordinating with the care team to support safe, effective in-home therapy services and positive outcomes for our patients.

Qualifications

• Bachelor's degree in Occupational Therapy.
• Current Michigan OT license in good standing.
• Current Michigan driver's license.
• Reliable transportation for daily use.
• Proof of automobile insurance.
• Ability to obtain BLS certification within three months of hire.
• Strong communication and interpersonal skills.
• Basic computer and mobile device proficiency.

Responsibilities

• Evaluate patients' functional abilities and develop individualized treatment plans.
• Deliver evidence-based occupational therapy services in the home setting.
• Help patients perform daily living activities safely and independently.
• Educate patients, caregivers, and family on treatment strategies and adaptive techniques.
• Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate care.
• Monitor patient progress and modify treatment plans as needed.
• Recommend and train patients on adaptive equipment, splints, and assistive devices when appropriate.
• Maintain accurate and timely electronic documentation.
• Participate in interdisciplinary care planning and process improvement initiatives.
• Serve as a clinical resource and mentor to students and support staff as needed.
