Job summary

Senior Care Solutions in Boca Raton, FL is seeking a compassionate Occupational Therapist (OT) to join our Home Health team on a per diem basis. You will provide in-home therapy to help patients regain independence in daily living and self-care while working autonomously in a patient-centered environment.

The role requires Florida OT licensure, at least one year of clinical experience (home health preferred), and ability to document care using WellSky EMR.

Qualifications

• Current FL licensure as an Occupational Therapist (OT).
• Minimum 1 year of clinical experience; home health experience preferred.
• Familiarity with WellSky EMR or similar documentation systems preferred.
• Strong clinical reasoning, time management, and communication skills.
• Ability to work independently and manage a flexible field-based schedule.
• Reliable transportation, valid driver’s license, and current auto insurance.

Responsibilities

• Perform initial and ongoing assessments of patients’ functional abilities, ADLs, mobility, safety, and home environment.
• Develop and implement individualized care plans based on physician orders and patient needs.
• Deliver therapeutic interventions including training in self-care, use of adaptive equipment, and home safety modifications.
• Educate patients and caregivers to support recovery and independence.
• Accurately document all care provided using WellSky.
• Coordinate care and communicate effectively with the interdisciplinary home health team.
• Maintain compliance with agency, state, and federal guidelines.
