FOX Rehabilitation is seeking a licensed Occupational Therapist to provide in-home and senior living care in New Bern, NC. You will conduct evaluations, create individualized care plans, and deliver evidence-based therapy to older adults in their homes and communities.

The role emphasizes autonomy, flexible scheduling, and the chance to work with an interdisciplinary team while incorporating telehealth when appropriate to improve access and continuity of care.

Formación

• Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Occupational Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsabilidades

• Provide one-on-one Occupational therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
• Document effectively within FOX’s EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
• Provide telehealth / virtual care when appropriate to extend access and flexibility
