Job summary

FOX Rehabilitation in New Bern, NC seeks a licensed Physical Therapist to deliver one-on-one care to older adults in home and senior living settings under the Geriatric House Calls model. You will evaluate, design and implement individualized plans, document in FOX’s EMR, and collaborate with an interdisciplinary team.

Qualifications

• Valid Physical Therapy license in NC or eligibility to obtain.
• Graduate of an accredited Physical Therapy program.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in home or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support mobility and independence.
• Document in FOX’s EMR according to regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and fitness staff.
• Educate patients and caregivers to promote safety and aging in place.
• Offer telehealth when appropriate to extend access.
