FOX Rehabilitation is seeking licensed Occupational Therapists to provide one-on-one care to older adults in private homes and senior living communities in Ramsey, MN. Therapists will establish individualized plans, deliver evidence-based treatment, and document care in FOX’s EMR, with collaboration across an interdisciplinary team.

The role offers autonomy, a flexible schedule, opportunities for telehealth integration, and care that improves mobility and independence in the home setting.

Qualificações

• Valid Occupational Therapy license in the state of practice.
• Degree from an accredited OT program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsabilidades

• Provide one-on-one Occupational therapy to older adults in homes or senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility, and independence.
• Document effectively in FOX’s EMR in line with standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
