Job summary

Qualifications

• Active Colorado OT license
• Experience in home health or related setting

Responsibilities

• Develop and implement individualized home health treatment plans
• Assess patients using standardized tools
• Deliver hands-on ADLs, mobility, and safety training
• Educate patients and caregivers on home programs and adaptive equipment
• Maintain accurate clinical documentation in EMR
• Collaborate with clinicians as part of a coordinated care team
