Job summary

You will play a vital part in helping older adults and individuals with disabilities thrive at home, using technology systems for efficient documentation. Expectations include minimum hours of 10 weekly and collaboration with a remote team. Reliable transportation is essential.

Qualifications

• 2+ years of clinical experience.
• Experience in home health, aging in place, or disability services preferred.
• Reliable transportation and valid driver’s license.

Responsibilities

• Conduct in-home assessments and functional evaluations.
• Deliver personalized, evidence-based recommendations for home modifications.
• Educate clients and caregivers on fall prevention and safe mobility techniques.
