Job summary

Humana's CenterWell Home Health is seeking a dedicated Occupational Therapy Assistant to provide in-home therapy services under supervision. You will implement treatment plans, design adaptive equipment, and educate patients and families to improve independence and function.

The role requires OTA licensure, CPR, and a valid driver's license with reliable transportation. Prior home health experience is preferred, with a pay range reflecting full-time hours.

Qualifications

• Current and unrestricted OTA licensure.
• Current CPR certification.
• A valid driver's license, auto insurance, and reliable transportation.

Responsibilities

• Provide therapy services planned, delegated and supervised by the qualified Occupational Therapist in accordance with the patient's Plan of Treatment.
• Assist in implementing vocational/education programs and activities established by the registered Occupational Therapist.
• Design/adapt equipment and working and living environments to improve function and independence.
• Provide therapeutic treatment and instruction to patients as directed by the Occupational Therapist and per physician orders.
• Document observations in patient records and assist with progress reports and required documentation.
• Instruct patients and family members regarding home programs and use of adaptive equipment.
• Participate in care coordination and discharge planning; support interdisciplinary team.
• Design community reintegration activities to aid physical reconditioning and psychological adjustment.
