Job summary

Powerback Rehabilitation is seeking an Occupational Therapy Assistant for in-home outpatient services in Laconia, NH. The role focuses on delivering occupational therapy under supervision to help older adults regain independence, in various home and facility settings.

The position offers a flexible schedule in multiple settings, with responsibilities including patient care, documentation, program development, and caregiver instruction. A valid state license and transportation are required.

Qualifications

• Must have a valid license in the state(s) of practice or proof of license eligibility.
• Must have a valid driver's license or reliable transportation to/from patients' residence.

Responsibilities

• Attends and contributes to patient care, staffing conferences and required meetings as a representative of occupational therapy.
• Adheres to department and patient treatment schedule; complete clinical and billing documentation and participate in meetings.
• Collaborates with rehabilitation team to develop programs aligned with patient needs and abilities.
• Instructs patients and caregivers in OT treatment techniques under supervision of the OT.
• Maintains timely documentation for all treated patients.
• Cleans, maintains, and reports equipment needs for repair.
• Puts Patient Service First by delivering high-quality, compassionate care.
• Implements individualized treatment plans as set by the OT.
• Provides OT interventions targeting cognitive, motor, balance, ADLs, safety, and home management; coordination and endurance.
• Performs other related duties as required.
