Job summary

Evolve Therapy Services in Milford, NE is seeking a compassionate Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility on a PRN basis. You will plan, organize, and direct occupational therapy services, conduct screenings, implement treatment plans, document appropriately, and collaborate with an interdisciplinary team.

Responsibilities

• Conduct regular resident screenings to determine intervention needs.
• Execute treatment plans and monitor progress per plan of care.
• Communicate evaluation findings and recommendations to residents and families.
• Establish therapy schedules with team members.
• Deliver occupational therapy services and ensure proper documentation and billing.
• Perform duties as assigned to treat patients effectively.
• Comply with CMS documentation standards in daily practice.
