Pathways Healthcare, a physician- and nurse practitioner-led home health and hospice provider in Londonderry, NH, seeks an Occupational Therapist to assess patients at home, develop individualized plans, and train patients in daily activities and adaptive equipment.

Qualifications

• Degree from an OT program accredited by AMA/AOTA and NBCOT-eligible.
• Minimum of two years of clinical OT experience preferred.
• Excellent verbal and written communication skills with strong organizational abilities.
• Maintain a current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Conduct comprehensive patient assessments, including functional status, home environment, and equipment needs.
• Evaluate work capacity and provide vocational training referrals when necessary.
• Develop and implement individualized treatment plans with achievable goals.
• Train patients in daily activities, prosthetics, orthotics, and adaptive equipment use.
• Maintain accurate records and report patient condition changes promptly.
• Educate families and caregivers on treatment plans.
• Supervise occupational therapy assistants and home health aides.
• Participate in training, meetings, and case reviews.
• Refer patients to additional services as needed and coordinate discharge planning.
