Job summary

FOX Rehabilitation is seeking a licensed Occupational Therapist to deliver one-on-one care in Highland Park, IL, including private residences and senior living settings. You will assess, plan, and implement individualized treatments to improve mobility, independence, and quality of life under the Geriatric House Calls™ model.

Qualifications

• Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Occupational Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one occupational therapy to older adults in homes or senior living communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility, and independence.
• Document effectively in the EMR following regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients and caregivers to promote safety and aging in place.
