Job summary

FOX Rehabilitation is expanding its Occupational Therapy team in Prescott Valley, AZ. We seek licensed OTs to provide one-on-one care in homes or senior living settings, with opportunities to incorporate telehealth for flexible coverage and continuity of care.

Qualifications

• Valid Occupational Therapy license in the state(s) of practice or eligibility to obtain.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one Occupational therapy to older adults in private residences or senior living communities.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy sessions to improve mobility, independence and safety in daily activities.
• Document care in FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote aging in place and safety.
