Job summary

FOX Rehabilitation in Lancaster, PA is seeking licensed Occupational Therapists to deliver one-on-one care to older adults in private homes or senior living communities. You’ll develop plans of care, provide evidence-based therapy, and document in FOX’s EMR. Telehealth may be incorporated to extend access and continuity of care.

This role offers autonomy to schedule your own visits and build strong patient relationships within a supportive, clinician-driven culture.

Qualifications

• Licensed Occupational Therapist in the state of practice (PA) or eligible to obtain license.
• Degree from an accredited Occupational Therapy program.
• Proficient with electronic medical records (EMR).
• Current CPR certification.

Responsibilities

• Provide one-on-one Occupational therapy to older adults in homes or senior living settings.
• Develop individualized plans of care and deliver evidence-based treatment.
• Deliver therapy sessions to improve mobility, independence and safety.
• Document sessions accurately in FOX’s EMR system.
• Collaborate with a interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers and staff to promote aging in place and safety.
