Signature Healthcare at Home is seeking a Home Health Occupational Therapist for King and Pierce counties in Washington. This part-time role offers a flexible schedule (Mon-Fri) and requires an OT license in WA, ACOTE-accredited education, NBCOT eligibility, CPR, and reliable transportation.

The OT will assess patients, develop and implement plans of care, educate patients and families, and coordinate with physicians and other health professionals to ensure high-quality home health services.

Qualifications

• Graduate from ACOTE-accredited OT program.
• Licensed to practice in Washington State.
• Eligible for NBCOT certification exam.
• Two years of clinical OT experience.
• CPR certification required.
• Must have reliable transportation/driver's license.

Responsibilities

• Implement standards of care for occupational therapy services per the plan of care.
• Collaborate with patient, family and caregivers to develop an individualized plan of care.
• Provide patient, caregiver, and family counseling and education.
• Communicate with physician and other health practitioners about the home health plan of care.
• Evaluate patient’s functional status and environment; identify equipment needs.
• Develop treatment programs and goals for improved function.
• Supervise assistants per policy and state regulations.
• Participate in discharge planning.
