Job summary

Rehab Without Walls® Neurorehabilitation in Walnut Creek is seeking an experienced Occupational Therapist to join our full-time Home & Community Neurorehabilitation team, delivering meaningful, real-life therapy in patients’ homes and communities.

You will serve as the primary therapist from evaluation to discharge, managing a 40-hour caseload with 5–7 patients per week and collaborating with an interdisciplinary team to promote independence in daily living.

Qualifications

• Minimum of a bachelor’s degree in Occupational Therapy from an accredited program.
• Current, unrestricted license as an Occupational Therapist in the practicing state.
• Current CPR Certification.
• Minimum of one year’s work experience as an Occupational Therapist.
• Knowledge of rehabilitation techniques related to complex neurological diagnoses preferred.
• Effective verbal and written communication.
• Ability to lift 50 pounds; moving or transferring patients up to 100 pounds with safety procedures.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly re‑assess effectiveness of treatment plans and participate in staffing meetings.
• Perform discharge evaluations and provide home/community safety recommendations.
• Communicate patient needs and progress to the treatment team and family.
• Collaborate with other disciplines to enhance patient outcomes.
