Qualifications

• BSN required.
• Experience as Director of Nursing or Nurse Supervisor preferred.
• NJ RN license and on-call availability as needed.

Responsibilities

• Create and monitor Plan of Care based on in-home evaluation.
• Oversee clinical quality and patient vital signs and care documentation.
• Provide in-home supervision of CHHA, home health aides and companions.
• Coordinate with care team to ensure patient safety and outcomes.
• Perform periodic patient evaluation and reassessment.
