Job summary

Giving Home Health Care is seeking a compassionate RN or LPN in Cordova, NM to deliver patient-centered in-home care under physician orders. This 1099 contract role involves 4 hours weekly with visits on Wednesday, Thursday or Friday, supporting patients in their homes and coordinating with CNAs, HHA, case managers, and physicians.

Responsibilities include executing care plans, documenting care, managing meds, assessing patients, and educating families.

Qualifications

• Graduate of an accredited college or university with nursing degree.
• Current State or Compact License as an RN or LPN.
• Current CPR certification.
• TB test and physical examination.
• Maintain required insurance.
• Good interpersonal skills.
• Hand-eye coordination and manual dexterity.
• Ability to stand or walk for prolonged periods and lift up to 50 lbs.
• Must have dependable transportation and commute to patient\'s home.
• Ability to provide medical management up to end of life care.

Responsibilities

• Execute physician prescribed plans of care.
• Complete compliant documentation of the care provided in the system of record.
• Manage and administer prescribed medication, treatment, and therapies.
• Conduct patient assessments, coordination of care.
• Perform activities of daily living.
• Monitor vitals and GI intake and output.
• Assess skin integrity and administer wound care.
• Monitor changes in patient condition (weight, self-care, disease progression).
• Communicate with family, physicians, case managers, and others.
• Provide education and training to patient/family/HHA.
• Work with patients one-on-one in home.
• Collaborate with and supervise CNAs and Personal Care Attendants.
• Perform other personal care services as needed.
