Job summary

Optum Home & Community Care is expanding its HouseCalls program in Missouri. As an Advanced Practice Clinician (NP/PA), you will perform in-home health assessments for Medicare Advantage members and other plan participants, focusing on education, gap closure, and collaboration with PCPs.

This field-based role requires active licensure in Missouri, certification in primary care, and the ability to work flexibly in patients' homes; generous compensation, loan repayment or sign-on bonus offered,

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted Missouri license by start date.
• Driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments for assessments.

Responsibilities

• Conduct in-home assessments for Medicare Advantage and other plan members.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to address care gaps.
• Identify urgent situations and intervene appropriately.
• Educate members on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals.
