Job summary

Qualifications

• Must hold valid Massachusetts NP license.

Responsibilities

• Conduct in-home health risk assessments including medical history review, vital signs, and physical examination.
• Perform medication reconciliation and identify diagnoses for care management.
• Collaborate with Porter Care Guides for personalized treatment planning.
