Job summary

Porter is seeking a Nurse Practitioner to visit members in the defined geographic area and perform in-home health risk assessments, including medical history reviews, medication reconciliation, vital signs, and comprehensive exams. You will identify diagnoses for care management and collaborate with Porter Care Guides to optimize treatment plans.

The role requires an active Massachusetts NP license and Medicaid Provider status, with EMR experience preferred.

Qualifications

• 3 years of patient care experience required (primary care/adult/geriatric)
• Valid unencumbered Massachusetts NP license
• Licensed as a Medicaid Provider
• Exceptional communication and empathy toward patients and coworkers
• EMR skills preferred

Responsibilities

• Visit members within your geographic area and conduct in-home health risk assessments.
• Evaluate past medical history, review systems, reconcile medications, take vital signs, and perform a comprehensive physical exam.
• Identify diagnoses for care management and assist in developing management plans with Porter Care Guides.
