Qualifications

• Active Massachusetts NP license.
• 3 years of patient care experience in primary/geriatric care.
• Experience with Medicaid preferred.
• Strong communication and empathy.
• EMR proficiency is required or preferred.

Responsibilities

• Conduct in-home health risk assessments and comprehensive physical exams.
• Review past medical history and perform medication reconciliation.
• Take vital signs and perform point-of-care testing as appropriate.
• Develop care plans with care guides and medical management for patients.
