Job summary

Advantmed is seeking a licensed Nurse Practitioner to conduct in-home wellness and risk-adjustment assessments on a PRN basis. The position offers flexible weekday schedules with no evenings or weekends and involves independent fieldwork in home settings.

Pay is ~ $110 per completed assessment plus mileage, with dedicated coordinators and training provided. This role supports value-based care initiatives across diverse patient populations.

Qualifications

• Active NP license in the applicable state.
• 3+ years patient care experience preferred.
• Comfortable with EMR systems and independent work.

Responsibilities

• Conduct in-home Annual Wellness Visits and chronic condition assessments.
• Perform histories, exams, vitals, medication reviews, and screenings.
• Educate members on preventive care and chronic disease management.
• Document visits in the EMR to support value-based care and risk adjustment.
