Qualifications

• Must have a valid unencumbered Nurse Practitioner License for the state you will be working in.
• Previous experience in in-home Risk Assessment is preferred.
• 3 years of patient care experience (primary care/adult/geriatric, EMR) preferred.

Responsibilities

• Perform annual wellness visits and health assessments on a population with chronic conditions.
• Deliver patient health education opportunities.
• Assist in closing quality care gaps (i.e. screenings and labs).
• Collaborate to enhance care delivery.
