Job summary

Monogram Health seeks compassionate Advanced Practice Providers to deliver in-home, evidence-based care for patients with multiple chronic conditions. You will join a multidisciplinary team and build long-term patient relationships through home visits and telehealth.

Roles include comprehensive assessments, patient education, and coordination with PCPs and specialists. Travel to patient homes is required; HIPAA compliance and strong documentation are essential.

Qualifications

• Active and unrestricted RN/NP or PA license required.
• Board certification in appropriate field (NP/AANP-ANCC or PA NCCPA).
• Current DEA certificate or eligibility to obtain.
• Minimum 2 years of direct patient care experience.
• Field-based role requiring home visits and travel.
• Knowledge of HIPAA and OSHA regulations.
• Experience with EHR systems (Athena, Salesforce) preferred.

Responsibilities

• Provide evidence-based, patient-focused care at patients' homes.
• Perform comprehensive assessments and wellness visits.
• Educate patients and caregivers on disease management and meds.
• Participate in recurring virtual integrated care team meetings.
• Coordinate care with PCPs, specialists, and ancillary services.
• Complete documentation accurately and timely.
