Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations, engaging with people in their own homes and helping bridge gaps in care.

You’ll use an iPad and a streamlined workflow for each evaluation, typically lasting about 45 minutes, with travel to local homes as needed and occasional virtual visits.

Qualifications

• Active, unrestricted licenses in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Responsibilities

• Conduct in-home health evaluations for health plan members.
• Perform medication and medical history review and physical evaluation.
• Utilize iPad workflow for each evaluation and adhere to clinical protocols.
• Travel locally and within licensed states, as needed; conduct virtual visits when required.
• Provide Diagnostic & Preventive Services per plan guidelines.
