Job summary

Signify Health, part of CVS Health, seeks a Part Time Clinician (NP or PA) to perform in‑home health evaluations at patients’ homes. The role supports bridging gaps in care with a patient‑centered approach and uses an iPad for streamlined clinical workflows.

The position is part‑time (approximately 29.5 hours/week) with a pay range of $46.03–$99.14 per hour. Responsibilities include health questions, history review, and optional diagnostic tests; travel within licensed states is required.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required. Multi‑state licenses preferred.

Responsibilities

• Provide in‑home health evaluations for health plan members.
• Engage with patients to review medications and medical history.
• Perform physical evaluations and, if ordered, additional diagnostic tests as needed.
• Coordinate with Signify Health services and workflows for home visits.
• Travel locally and within licensed states as needed; conduct virtual visits when required.
• Ensure patient safety and privacy during home visits.
