Qualifications

• Home health experience (Preferred)
• 1 year of in-home assessment experience (Preferred)
• Current RN and NP or PA licensure in the present state of practice

Responsibilities

• Provide annual in-home assessments for Medicare and Medicaid members
• Document and submit chart notes within 24 hours
• Attend periodic training sessions and meetings

Skills

Empathy in patient interactions

Ability to make critical decisions

Proficiency with electronic medical records (EMR)

Education

Master’s or Doctorate degree from an accredited NP or PA program
