Job summary

Emcara Health is seeking a Per Diem Nurse Practitioner or Physician Assistant to conduct in-home assessments for underserved patients in Clinton, Maine. The ideal candidate will provide critical health assessments that identify care gaps and health risks.

This position offers a flexible schedule and compensation rate of $150 per completed assessment, alongside performance bonuses. Applicants must have current licensure and advanced education in nursing or physician assistance.

Qualifications

• Home health experience preferred.
• 1 year of in-home assessment experience preferred.
• Current RN and NP or PA licensure.

Responsibilities

• Provide annual in-home assessments for Medicare, Medicaid, and ACA members.
• Document and submit chart notes within 24 hours.
• Perform high quality assessments to meet patient needs.

Skills

Empathy in patient interactions

Critical decision-making

Understanding of risk adjustment models

Proficiency with electronic medical records (EMR)

Ability to engage with new patients

Education

Master's or Doctorate degree from accredited NP or PA program
