Job summary
Monogram Health is seeking compassionate Advanced Practice Providers (NP/PA) to deliver high-quality in-home care to patients with multiple chronic conditions, including diabetes, hypertension, CKD, heart failure and COPD. The role involves home visits and telehealth within an integrated care team to support a value-based, patient-centered model.
Base salary ranges from $122,000 to $158,600 with additional bonuses; mileage reimbursement and CEU/CME expense reimbursement are provided to support
Qualifications
- Active and unrestricted NP or PA license.
- Board certification (NP: ANCC/AANP | PA: NCCPA).
- Current and unrestricted DEA certificate.
- Minimum of two (2) years of direct patient care experience.
- Field‑based role requiring travel to patient homes, reliable transportation, valid driver’s license, and auto insurance.
- BLS certification (supported during onboarding)
- Knowledge of and experience with EHR systems (Athena, Salesforce, Epic)
Responsibilities
- Provide home-based, patient-focused care to reduce avoidable hospitalizations.
- Perform comprehensive assessments and wellness visits.
- Educate patients on disease management and preventive care.
- Participate in recurring virtual integrated care team meetings.
- Coordinate care with PCPs, specialists, and ancillary services.
- Complete documentation accurately and in a timely manner.
