Job summary

Monogram Health, Inc. is seeking Advanced Practice Providers (NPs or PAs) to deliver high-quality in-home care to patients with multiple chronic conditions, including diabetes and CKD.

The role integrates with a multidisciplinary team and includes telehealth visits, with no on-call requirements. Base salary ranges from $122,000 to $158,600, with opportunities for bonuses, mileage reimbursement, and CEU/CME expense reimbursement.

Qualifications

Active and unrestricted RN and NP licenses or PA license.
Board certification required (NP ANCC/AANP; PA NCCPA).
DEA certificate active or eligible.
Minimum 2 years direct patient care.
Field-based role requiring travel to patient homes.

Responsibilities

Provide evidence-based, patient-focused care in the patient’s home to reduce hospitalizations and improve quality of life.
Perform comprehensive assessments including wellness exams and post-discharge visits.
Educate patients and caregivers on disease management and preventive care.
Participate in virtual integrated care team meetings.
Engage in quality assurance activities and ensure compliance with state boards.
