Job summary

CVS Health, through Signify Health, seeks a Full Time Clinician (NP/PA) to perform in-home health evaluations across designated coverage areas. Visits are about 45 minutes, including medical history, evaluation, and plan alignment with the health plan.

You will travel locally and within state licenses, using an iPad-based workflow, and provide diagnostic and preventive services as required. Active licenses and board certification are required, with multi-state licensure support.

Qualifications

• Active, unrestricted license in coverage area(s)
• Board certification (required)
• Able to obtain and maintain multi-state licenses in coverage area, as required
• Able to cover all counties within state and flex to other areas of business need

Responsibilities

• Conduct 45-minute in-home evaluations with patients
• Perform medication and medical history review and physical examination
• Utilize iPad and clinical workflow for each evaluation
• Travel locally and within licensed states; up to 100% travel
• Conduct virtual visits as needed
• Provide Diagnostic & Preventive Services (spirometry, DEE, etc.) per health plan orders
