Job summary

CVS Health, including Signify Health, is seeking a Full-Time Clinician (NP/PA) to perform in-home health evaluations for plan members across communities. You will engage with patients in their homes, review medical history and conduct physicals using an iPad-based workflow, without prescribing medications or ordering labs.

The role supports a national network of clinicians, requires an active license in coverage areas, board certification, and willingness to travel locally.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Responsibilities

• Join national network of clinicians to visit members in their homes.
• Conduct in-home health evaluations with symptom review, history and physical.
• Utilize iPad and clinical workflow during evaluations without prescribing or ordering tests.
• Travel locally within licensed states as needed and conduct virtual visits when required.
• Provide Signify Health services including diagnostic and preventive assessments.
