Job summary

Signify Health, part of CVS Health, seeks a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations, meeting members at their homes and bridging gaps in care.

Visit lasts about 45 minutes, including medication history, physical evaluation, and optional tests; uses an iPad with a simple clinical workflow. You are not required to prescribe or alter treatments. 29.5 hours weekly; pay range shown; Idaho-based with travel within licensed states.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).

Responsibilities

• Perform in-home health evaluations for health plan members.
• Engage with patients to bridge gaps in care and answer health questions.
• Conduct visits using iPad with clinical workflow; may include diagnostic tests as indicated by the plan.
