Signify Health, part of CVS Health®, seeks a Part Time Clinician (NP or PA) to provide in-home health evaluations. Visits occur in members’ homes, connecting them with the right care and bridging gaps in care.

The role involves 29.5 hours per week, with travel within licensed states and virtual visits as needed, using an iPad and a clinical workflow. Board-certified, licensed clinicians are required.

Formación

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsabilidades

• Provide In-Home Health Evaluations for health plan members.
• Conduct 45-minute visits including med history and physical evaluation.
• Travel locally and within licensed states as needed.
• Utilize iPad and clinical workflow; no prescribing or ordering routine tests.
