Job summary

Signify Health, part of CVS Health®, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations. Your visits occur in patients' homes, with a focus on bridging gaps in care and connecting members to the right services.

The role includes a ~29.5 hour weekly schedule, with duties spanning diagnostic and preventive services, and virtual visits as needed. Licensed NPs or PAs with board certification are required.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).
