Job summary

Signify Health, a CVS Health company, seeks a Part Time Clinician (NP or PA) to perform in-home health evaluations for plan members. You’ll connect with patients in their homes, review medical history, perform a physical evaluation, and support personalized care coordination.

You will travel locally and within licensed states, conduct virtual visits as needed, and provide diagnostic services as indicated by health plans. Strong licensure and board certs are required.

Qualifications

• Active, unrestricted NP or PA license in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsibilities

• Conduct in-home health evaluations with patients.
• Collaborate with health plans to bridge gaps in care.
• Travel locally to visit members in licensed states.
• Perform documentation and clinical workflow via iPad.
