Signify Health is seeking part-time in-home clinicians to join a national network delivering diagnostic and preventive services at members’ homes. You will conduct visits, review medical history, perform selective assessments, and coordinate with health plans to connect members with the right care.

Licensed clinicians with board certification, active unrestricted licenses in the coverage area(s), and multi-state credentials are preferred.

• Contribute to the goal of visiting millions of people nationwide and connecting them to the right care for their unique needs.
• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
• Be flexible to travel locally and within licensed states as needed.
• Conduct virtual visits as required.
• Provide all Signify Health services indicated, including Diagnostic & Preventive Services products such as spirometry, diabetic eye exams, and bone density screening.
