Job summary

Signify Health, part of CVS Health®, is seeking a Full-Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

The role involves 45‑minute visits to review medications and medical history, perform a physical evaluation, and conduct additional diagnostic tests as ordered by health plans. You may travel locally within licensed states or conduct virtual visits as needed.

Qualifications

• Active, unrestricted license(s) in coverage areas
• Board certification (required)
• Multi-state licenses (preferred)

Responsibilities

• Be part of a national network of clinicians dedicated to improving health outcomes.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Signify Health services including diagnostic & preventive tests like spirometry and other screenings.

Education
