Job summary

Signify Health, part of CVS Health, seeks a Per diem Clinician (NP or PA) to provide in-home health evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. The role offers part-time hours (12 per week), requires active, unrestricted licenses in coverage area, board certification, and multi-state licenses are preferred.

Travel locally and within licensed states and virtual visits as needed.

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsibilities
