Job summary

Optum Care at Home seeks a Nurse Practitioner to deliver longitudinal, hands-on care to high-risk patients in their homes across multiple states. You’ll work with an interdisciplinary team to prevent hospitalizations and improve quality of life, with strong autonomy and patient relationships.

Qualifications

  • Active ANCC or AANP certification (Family, Adult, Geriatric, or Adult-Gerontology Primary Care).
  • Active, unrestricted state NP license.
  • DEA license/prescriptive authority (or ability to obtain).
  • Ability to obtain collaborative practice agreement if required by state law.

Responsibilities

  • Conduct comprehensive in-home assessments and medical history reviews.
  • Manage acute illnesses and chronic condition exacerbations.
  • Provide care during short-term rehabilitation episodes.
  • Document diagnoses and ICD-10 conditions accurately.
  • Collaborate with interdisciplinary care teams and lead discussions on advanced care planning.
  • Educate patients and caregivers on treatment plans and disease management.
  • Coordinate care and support services to improve outcomes.
